Congressman Chris Smith Visits CHEMED in Recognition of Its Achievement in Attaining National Level 3 PCMH Status

Chris Smith, U.S. Congressman for New Jersey’s 4th district, recently visited the Center for Health, Education, Medicine and Dentistry (CHEMED), a federally qualified health care center, 1771 Madison Avenue, Lakewood, to congratulate officials there for being recognized at the highest level as a Patient Centered Medical Home (PCMH) by the National Council on Quality Assurance (NCQA). PCMH puts patients in the forefront of care.

After a presentation depicting the growth of CHEMED and a tour of the facility, including CHEMED’s newly built out space that will house CHEMED’s innovative Care Management Program, officials cited the positive contributions Congressman Smith has made in support of the Health Center by presenting Congressman Smith with a plaque that read, “In recognition of Congressman Chris Smith who has supported the LRRC/CHEMED Health Center since its inception and recognized its mission to serve the healthcare needs of Lakewood and the surrounding communities. Your vision enabled CHEMED Health Center to achieve level 3 PCMH recognition.”

Dr. Dovid Friedman, CEO of CHEMED said, “CHEMED strives to meet the medical needs of Lakewood and the surrounding populations by providing quality, patient centric healthcare to all.  CHEMED has serviced over 70,000 unique patients in over 850,000 visits.  CHEMED’s services include internal medicine, geriatrics, women’s health, pediatrics, podiatry, specialty, dentistry, radiology, behavioral health, as well as a fast track/immediate care department.”
