CHEMED Recognized at Highest Level for Patient Care
The Center for Health Education, Medicine and Dentistry (CHEMED), a federally qualified health care center, 1771 Madison Avenue, Lakewood, N.J., has been recognized at the highest level as a Patient Centered Medical Home (PCMH) by the National Council on Quality Assurance (NCQA).
The NCQA’s Patient-Centered Medical Home Recognition Program, the most widely adopted evaluation program for physician practices in the country, evaluates PCMH as a model that puts patients in the forefront of care. For example, CHEMED focuses on patient treatment that is coordinated through a primary care physician to ensure patients receive the necessary care when and where they need it, in a manner they can understand.
Some of the advantages of PCMH at CHEMED include an integrated care model that emphasizes that a patient’s health is not just about primary care. CHEMED doctors and staff care for the entire person, from helping patients navigate their financial assistance options to preventive screenings and services.
