CHEMED Recognized at Highest Level for Patient Care

The Center for Health Education, Medicine and Dentistry (CHEMED), a federally qualified health care center, 1771 Madison Avenue, Lakewood, N.J., has been recognized at the highest level as a Patient Centered Medical Home (PCMH) by the National Council on Quality Assurance (NCQA).

The NCQA’s Patient-Centered Medical Home Recognition Program, the most widely adopted evaluation program for physician practices in the country, evaluates PCMH as a model that puts patients in the forefront of care. For example, CHEMED focuses on patient treatment that is coordinated through a primary care physician to ensure patients receive the necessary care when and where they need it, in a manner they can understand.

Some of the advantages of PCMH at CHEMED include an integrated care model that emphasizes that a patient’s health is not just about primary care. CHEMED doctors and staff care for the entire person, from helping patients navigate their financial assistance options to preventive screenings and services. Communication with patients, their families or caregivers give enhanced patient access to clinical advice and medical records.

CHEMED also provides better management of chronic conditions, such as diabetes, and focuses on helping patients stay compliant and achieve individually designed care goals. Flexible scheduling and fast track appointments, and an advanced electronic medical records system and portal, allow for secure patient communication, along with reminders and follow-ups on referrals.
